If you are not sure where to seek medical care when you become sick or injured, call us FIRST. It is always best to first call your primary care provider during and after hours before going to the emergency room.

Call 911 or go to the nearest emergency room. You do not need to call us FIRST because a delay in seeking immediate medical attention could be hazardous to your life or health.

 

 

To schedule an appointment at one of our clinics, please call (901) 261-2000. We will make every effort to schedule your appointment with your primary care provider every time you need to be seen.

Yes, you should arrive 15 minutes before your scheduled appointment time to allow adequate time to complete any paperwork and insurance information.

You need to bring your insurance information, a copy of your driver’s license, co-payment or deductible, and all medications you are currently taking.

If you are more than 30 minutes late for an appointment, you will be triaged by a nurse. Based on the results, you may be worked in as a walk-in and seen as soon as the schedule allows, offered a same-day appointment, or asked to reschedule your appointment.

If you call to request a refill and your last medical visit is less than six months, the provider may agree to call in enough medication to last until you can schedule an office visit. If your last medical visit is more than six months, to ensure not only the safety or effectiveness of your medications but also to ensure you are receiving the best possible care, you will be asked to schedule an appointment with your primary care provider. It is your responsibility to schedule an appointment before you run out of medication.

Controlled substances, including some medications for pain and anxiety, will only be refilled during an office visit with your primary care provider and cannot be refilled automatically or by another primary care provider.

Lab results vary in the time they come back to your primary care provider. You will be contacted by telephone and/or mail when your lab results are available.

You can request a copy of your medical records by signing an authorization for release of information form. If you are not the patient, parent, or legal guardian, then you must obtain written permission from the patient.

If you want a copy of your medical records sent to another health care provider, attorney, or insurance company, an authorization for release of medical records form must be completed and signed. Once the approved form is received, a copy of your records will be sent to the requesting party.

Please allow up to 14 business days for your request to be processed. There is a minimum fee for this service.

Patients are encouraged to make appointments. Although we will evaluate your condition if you walk-in without an appointment, you may have a longer wait time since patients with appointments will be seen first.

You should not expect to walk-in for care of chronic health conditions like diabetes or high blood pressure management, refills, or routine physical and wellness exams.

Scheduling an appointment is always your BEST OPTION. By scheduling an appointment, you reduce your wait time and get an appointment time that’s convenient for you.
