POSITION SUMMARY:

The COVID-19 Case Manager will be responsible for assisting with planning, organizing, coordinating, monitoring, and evaluating assigned elements of the company’s COVID-19 program.

ESSENTIAL DUTIES/RESPONSIBILITIES:

v Identifies gaps or barriers in treatment plans making outbound calls to assess COVID-19 patients.

v Educates and counsels patients regarding transmitted diseases and risk reduction.

v Provides pre-screening of individuals calls for COVID-19 testing and post-testing follow-up with results or contact questionnaire.

v Interviews patients to elicit information regarding their contacts to COVID-19 positive patients and to determine possible personal and health history related to transmitted diseases.

v Directs these individuals to submit necessary laboratory specimens and such other necessary control measures.

v Performs a variety of duties relating to the preparation of reports, entering personal data on patient registers and preparing correspondence to send to patients who are in need of medical examination and treatment.

v Tracks all COVID-19 test results. Maintains records on all delinquent and outstanding test results.

v Gathers data on all COVID-19 tested individuals and activity reports on a weekly, monthly, and quarterly basis. Actively participate in COVID-19 testing activities.

v Performs other duties as may be requested to assist in keeping the organization compliant with all applicable standards (State, Federal and Joint Commission).

EDUCATION/EXPERIENCE:

v Certification of practice as a Licensed Practical Nurse in Tennessee with 2-3 years of experience in clinical or community resource settings; Care coordination in an outpatient setting experience is desirable.

v Demonstrated proficiency in multi-tasking, flexibility, time management, organization, attention to detail, scheduling, professionalism and quality focus. Functions well in a team focused environment.

Demonstrated computer skills including but not limited to: MS Word, Excel and Outlook.
